Cardiac Rehabilitation

At The Phoenix Center, we pride ourselves on our state-of-the-art Cardiac Rehabilitation Program, designed to support individuals on their journey to recovery following cardiac events or procedures. With a proven track record of enabling hundreds of patients to lead longer, healthier, and more fulfilling lives, our program stands as a beacon of hope for those in need of specialized cardiac care.

Personalized Care from the Start

Upon joining our program, each patient undergoes a detailed evaluation to assess their specific medical needs, laying the groundwork for a personalized rehabilitation plan. This plan is crafted in collaboration with a multidisciplinary team of healthcare professionals, including registered nurses, pulmonologists, and physiologists, ensuring a comprehensive approach to cardiac recovery.

Tailored Rehabilitation Strategies

Our approach to cardiac care is dynamic and patient-focused. We utilize the latest in medical treatments and therapeutic techniques to address the unique challenges faced by individuals recovering from heart attacks, transplants, angioplasty, congestive heart failure, and other cardiac conditions. Our goal is to not only improve heart health but also to enhance overall well-being and quality of life.

Continuous Support and Communication

Throughout the rehabilitation process, we maintain open lines of communication with families, providing regular updates on their loved one's progress and addressing any questions or concerns that may arise. We believe that family involvement is crucial to the success of our patients’ recovery journeys.

A Team Dedicated to Your Heart Health

Our knowledgeable and compassionate team is committed to putting each patient's rehabilitation plan into action swiftly and effectively, ensuring that every aspect of their cardiac care is managed with precision and empathy. At The Phoenix Center, we are dedicated to helping our patients achieve optimal heart health and to supporting them every step of the way.

Note that not all the quality measures that are reported on Care Compare are included in the rating calculations. In addition to an overall quality measure rating, separate QM ratings for short-stay measures and long-stay measures are also reported.
