Today, The Phoenix Center is proud to be recognized as a leading 227-bed skilled nursing facility, renowned for its innovative care and specialized programs.

This commitment to ambiance, paired with our advanced vent systems and alarm technology, underscores our dedication to providing a nurturing, state-of-the-art environment for our patients and their families.

Specialized Programs and Services

Our journey of innovation is highlighted by the introduction of a pioneering peritoneal dialysis program, spearheaded by Dr. Kenneth Lieberman. This program reflects our commitment to employing advanced technology in diagnosing, treating, and managing a wide range of kidney and urinary tract disorders, setting new standards in patient care.

A CMS 5-Star Rated Facility in Quality Measures

Ratings for the quality measures are based on performance on 15 of the QMs that are currently posted on the Care Compare website. These include nine long-stay measures and six short-stay measures. Note that not all the quality measures that are reported on Care Compare are included in the rating calculations. In addition to an overall quality measure rating, separate QM ratings for short-stay measures and long-stay measures are also reported.
